Can I bill 64626 and the add on code 64627 per cervical level (e.g., C2,C3,C4,C5)? Would this be the correct way to report?

I would bill that as:

64626
64627x3

and never had any problems with it. Really depends on the payer's preference. I've never used multiple lines with mod 59, but there are many others who have posted in other threads that your way is the to do it.
