dr is performing a 20680 (removal of implant) normally when its in the post op you add mod 58, this is 3 days out of post op, should I not add it, since its out of post op, also, since its out of post op wouldn't our payment be like 15 percent higher? does anyone know, thank you
 
Coding depends on if the implant was meant to come out or not

Your coding will depend on whether the physician knew that the implant would need to be removed or not. Some hardware you know will be taken out at a future date, for sure. Other times, the hardware may stay in the patient as long as it does not cause any problems. Look at when the implant was done. If they knew that the implant would need to come out, modifier -58 would be appropriate. Modifier -58 should not reduce your payment.
