We are being told that for NGS Medicare, Definitive Quantitative UDT should use special billing (see below), but I can't find anything from NGS to document this.

1) For code 80299 it needs a ?NOC? followed by name of the assay where do I but it on the claim ??

2) Class Codes can only be billed with Quantity of two for example like Opiates which G codes are considered Class codes ??

3) Which codes would need a 3 alpha character added

Is anyone billing this to NGS Medicare? We want to make sure we do it right - not just get paid.
