Here is my scenario: The physician prepares the vial before the patient comes in for the injection. The patient NEVER shows up for the injection and NEVER returns to the practice and now the vial is expired. Are we allowed to bill the 95165 for prep of the vial even though it was not used? Thanks

Yes, you are allowed to bill for the vial charge 95165... you need to be paid for the serum and the work of mixing it....the pt fails to tell you that they want to cxl the mix...they are still responsible and they insurance/patient should be billed.
