A patient comes in with Chest pain and regurgitation the doctor assess the patient and the dx code arthritis ? should I code the chest pain and regurgitation?

I would code the chest pain if that is the cheif complaint. Also, is this cardiology? If so, I would definately add the chest pain and regurgitation and put the arthritis secondary. Arthritis just might be an observation that the physician is just noting in the documentation.
