What diagnosis do you use if the report just says placement for a perm cath for hemodialysis? Normally I get ESRD or chronic kidney disease etc... Would I use Z45.2 or is there a better Z code to use just for this type diagnosis?

Z49.01 Encounter for fitting and adjustment of extracorporeal dialysis catheter could be used. There's a notation to "Code Also associated end stage renal disease", but since the ESRD is not documented, I probably wouldn't tack on N18.6 even though it's pretty obvious that's the situation.
