Wiki QW Modifiers

We are a physicians office with a certificate of registration from CLIA to do moderate complexity tests in our in-house lab. Do we still have to use QW modifier on all tests that are CLIA waived and not use it on the ones that aren't? What about the moderate complexity tests? I am confused! Thanks

If you go to www.cms.gov and type in the search box CLIA waived tests, you will be taken to a document that will show you the tests that require a QW modifier.
