Wiki QZ modifier & NPI question

Scenario- The surgeon, who is also an anaesthesiologist is performing a level 2 surgery. His CRNA performs a 01992. The physician is NOT monitoring the anaesthesia. How is this billed? Does it need to be billed out under the CRNA NPI? Is the physician considered supervisory when he is NOT supervising the anesthesia but is in the room performing surgery?
