If R50.9 fever, unspecified is all that I have as the diagnoses, what do I do if it is being denied?

Will it always need a supporting code to justify?? If the provider do no list any other dx, should I query him/her for an additional dx always?

What does the denial state? What visit level did you submit? Were there any other codes on they claim?
