Wiki Radial and ulnar thromboembolectomy

Our physician performed a thromboembolectomy (procedure code 34111) on a patient. He removed clots from the patient's radial and ulnar arteries. This code reads radial or ulnar. We are trying to decide if we can bill this twice for each artery. Does anyone have any documentation showing where this can be billed for both arteries? Or, should it only be billed once?
