Should 77470, Special Treatment Procedure, be billed on patients that are receiving Oral chemotherapy vs IV chemotherapy, concurrently with radiation?
 
No. CPT 77470 is only supported for concurrent IV chemotherapy, in which it required above and beyond the norm planning for treatment by the radiation oncologist MD. Per ASTRO/ACRO and NIA: A special treatment procedure (CPT 77470) may be utilized for circumstances that require extra and inordinate amounts of time and effort by the staff and physician, which is medically necessary for the patient and not routine to the service being performed. If patient circumstances require additional planning or treatment time by the radiation oncologist, in addition to an IMRT course of therapy, CPT 77470 may be billed. The standards are: Special Treatment procedure must be requested by the radiation oncologist provider, a max quantity of 1 unit is allowed per course of treatment, when requested in conjunction with IMRT, SBRT, SRS or concurrent chemotherapy one unit may be aproved. It is NOT authorized for the sole use of 3D conformal or IMRT, it may be approved for cirumstances above and beyond the routine planning and treatment of these modalities. It will not be routinely allowed merely for the receipt of concurrent chemotherapy, but instead be based on documented medical necessity requiring extensive work on the part of the physician and staff as a result of the chemotherapy in combination with radiation therapy. When it is requested with 2D, 3D or IMRT courses of radiation therapy, patient specific medical necessity rationale is required. CPT 77470 may be approved if ALL of the following criteria are met: 1-rationale is supplied by the radiation oncologist, 2-it has not been previously preauthorized with the same course of treatment therapy, 3-the rationale explains that additional time and effort will be incurred for the patient in question, above and beyond routing planning and treatment of 2D, 3D, or IMRT courses of therapy. Requests not identified as meeting the criteria require a peer to peer physician review pre-service. I hope this helps!
