Our doctor performed the following:

Right L5 and S1 medial branch rfa
Right S1, S2, and S3 lateral branch rfa

He codes his own procedures generally and put 66640 (??) on this note. I say that he means 64640. My question is do I just bill it once or twice (once for medial and once for lateral)?

Ashley, I am not sure that 64640 is the correct code for Lumbar and Sacral procedures? I use 64622 and 64623 for add lesions. Let me know what you think?

I code for an ortho practice- and to determine the code, you need to know the type of nerve being treated. I normally code 64622, 64623. Since it's not bilateral- I'd put RT as a modifier to distinguish which side. Hope this helps!
 
Thanks for your help. I asked our reg admin and she coded it a different way but she isn't a coder. The Dr will be back from vacation on Monday and I will talk to him about it.
