RE: Is this coding scenario correct?

A7004
J7620
99214-25

Seems the J7620 for $50.00 denies all the time. Is this the correct way to code? Was thinking I need to add the J7620 with the office visit and changed the office visit code to support the code. Anyone please?

A7004: HCPCS for Disposable nebulizer sml vol

Descriptor: Small volume nonfiltered pneumatic nebulizer, disposable
HCPCS Guidelines
Section Specific Guideline
C codes are used ONLY as a part of Hospital Outpatient Prospective Payment System (OPPS) and are not to be used to report other services.

J7620: HCPCS for Albuterol ipratrop non-comp
Descriptor: Albuterol, up to 2.5 mg and ipratropium bromide, up to 0.5 mg, fda-approved final product, non-compounded, administered through dme

In my opinion, you have the coding sequence backwards as the E&M (99214) should be the first line with second line for a nebulizer treatment, CPT 94640: AIRWAY INHALATION TREATMENT and then with the medication.
