I'm not sure what ICD-10 code to use for EKG done in office that turned out to be abnormal. Do I use the reason for the EKG (extremely high BP reading/HTN), or do I use the result of the EKG?

Pt presents with hypertension. Reading is 242/110. EKG ordered and completed, showed abnormal results. Do I code as:

I10, 93000
R94.31

or

R94.31, 93000
I10

Which code goes with the 93000?

Along this same line of questioning, is it correct to code the result of a pregnancy test with the test when it is available that same day, or do I code the reason for the pregnancy test (e.g., Z32.00) and the result (positive or negative) as secondary code?
