Good morning everyone,
Patient was seen by the provider for regular chronic conditions (DM,HTN,etc.,) and due to patient's previous historyprovider ordered for stool test .
2 days after NP called patient to come and get the RX for of H.pylori.
Is it Ok to code 041.86 (H.pylori) with CPT 99211. Patient came to clinic to pick up RX and provider documented in A & P (lab results) . NO Exam and HPI
If it is already included in previous E/M code then how/when do you inform the patients's condition and treatment to the insurance company??
