Recent changes to covering dx for 85025

Hello, has anyone experienced recent changes with the dx that will/will not pass medical necessity for 85025? All of the sudden the passed month or so i'm having issues with this test not passing medical necessity. I am still comparing my accounts to try to figure out maybe which dx covered previously but does not anymore, but figured I would as here as well.
