pt had a colonoscopy in 2007 and had rectal polyps. does this code to 569.3 or V12.72? note does not say if the polyps were previously removed. thanks!
 
V12.72; the pt had polyps; if it was found and documented in 2007, means already has a history of polys, no matter if it were removed or not...hope this helps!
