Need some advice on this - the patient had a distal rectal ulcer ablated with CO2 laser. Would anyone consider using 45190, destruction of rectal "tumor" for this? Or, since only an anoscope was used, could this be 46615 (not stated as anal, but distal rectum??). I have one coder using 45190 but I think 46615 might better apply.

Also, is the CO2 laser considered to be thermal energy? This time the phys did not specify excision by laser, but just ablasion (destruction).
 
There wasn't a reason for using ablation given. Just for removal I'm guessing.
