I am in Washington state. Is there a rule or guidelines for sending patient refunds? Is there an amount where we don’t have to refund...like .50? Also what is an office to do if the refund is returned because the patients address has changed and can’t be contacted?

I would check with your state's department that handles unclaimed funds. Most likely you'll need to turn any unclaimed patient refunds over to them. (If the check still isn't cashed after you have done your due diligence and attempted to send a refund check to the patient.)
