Hi friends, i need a help from experts that in BCBSTX, that a patient does the procedure for Occipital nerve block bilateral with (Lesser) & (Greater). So keyed in 64405 50 ;51 & 64405 50 ; 51. Is it correct? the claim gets rejected. Do we need to add any other modifiers? Date of service is 07/28/2020. Please help me out as soon as possible.

For the second procedure code, CO-222 (Exceeds the contracted maximum number of hours/days/units by this provider for this period). I am new to this Billings side.. slowly getting into deeper..
