I have a patient that was seen via telemedicine by a provider to discuss if they met the criteria to receive the monoclonal antibody injection - Regen Covid. The patient lives with his wife and daughter who both have COVID-19 so he was exposed to the disease. I know I need to use dx code Z20.822 but this isn't a primary dx code. The visit note states the patient has no symptoms at this time and has not tested positive for COVID-19. I'm unsure if there is a dx code I can use based off the encounter note from the physician. Any advice would be appreciated.

Z20.822 is valid as a primary diagnosis code - that is appropriate to use in this situation. I'm not sure why the provider is administering this therapy to a patient without a positive test though - I didn't think it was the standard to use this as a preventive measure for a patient without a positive test result, so it wouldn't surprise me to see it deny.
