Hello!
Is there anyone having modifier issues with Humana in regards to billing for TOB 222? I billed TOB claims 222, 223, 224, 232, 233 and 234 claims for Revenue Codes for 420, 430 and 440. With Humana, some 97530 -59 are also billed. I bill on UB04 claim and may have 59 line items. There is a claim where all the line items denied for modifier issues. Am I supposed to use a KX modifier as well. I've reached out to Humana and all they can tell me is that they can sent the claim back for review. I think I could sent the corrected claims if I had an answer. I billed for Rehab claims in a nursing facility.
