Remission confusion

I have a chart with a Dx of
204.01 - Acute Lymphoid Leukemia in Remisson

with a surgical procedure of
96450 - Lumbar Puncture for Chemotherapy

My confusion is how the physician states that the patient is in remission yet they are still receiving active treatment. If you are being treated for the condition doesn't that mean you still have the condition?
 
So shouldn't my dx then be for active Leukemia and not for Leukemia in remission?
