Remote Patient Monitoring Billing

Hello,
Does anyone have experience billing for remote patient monitoring?
I work for an orthopedic medical office and our providers prescribe a device for remote patient monitoring.
The basic codes provided by the device manufacturer include 97161 with GP modifier, 99453, 99454, and 99457.

Can you bill 97161 on the same day as an outpatient E/M code (ex: 99214)?

For 99457 - The chart review is completed by a team member of the manufacturer (not from our office), but advised to bill under our MD for reimbursement as incident-to.
Can you bill incident to for another qualified health care professional (LVN for example) who does not work for your office?
