Wiki Removal of Jacob Ladder

It is my understanding if no incision was made and he just simply took one end of the piercing off and pulled it out, it would be included in the E/M service. Did the physician do anything to the site afterwards?
 
PROCEDURE PERFORMED
Removal of Jacob ladder.
Debridement.
Undermining flap advancement closure, 8 cm, right forearm

DESCRIPTION OF PROCEDURE
The patient was brought to the operating room. After informed consent,
confirmation of site, time-out protocol, he has been on timed antibiotics, hand
and arm prepped and draped in the appropriate sterile manner after anesthesia.


Under loupe magnification, the Jacob's ladder was removed. The wound was
examined, debrided non-excisionally and undermined flap advanced. It was able
to be closed without undue tension with Monocryl, with no untoward effects.


The wound was covered with bacitracin, Adaptic, fluff dressing, bulky dressing
and Kling. Hemostasis was controlled with bipolar cautery. The patient
tolerated the procedure well, left the operating room in satisfactory
condition.
