Hello,

We have several patients come into our Family Practice clinic for lipoma removals. We just came across an older article from 2014 and just want to make sure this is still correct information. Per this article a lipoma removal procedure code should come from the Musculoskeletal System NOT the Integumentary System. Is this correct?

Sep 2018 CPT Assistant 'addressed' (ha) the issue but unfortunately never mentioned lipomas. Only mentioned sebaceous cyst removals should be coded with the integumentary codes. I'm still following the Aug 2006 CPT Assistant that says to code lipoma removals with musculoskeletal codes unless they are in a superficial location, then use integumentary codes.
 
Agree to use musculosketal codes but watch your payers. Several of these CPT codes have a CMS ASC indicator of G2 meaning that office is not a valid POS for billing. We have had BCBS denying our claims as of 4/1/19.
 
IMHO, code selection will depend on where it is located and the depth. I would choose a code in the Musculoskeletal System only if it is located below the fascia.
 
Look in the CPT vignette; excision of subq tissue tumors: resection of tumors confined to subq tissue below the skin but above the deep fascia.
