Our pediatricians have done these several times in office as long as there is not bone involvement. We bill CPT 11200 w/dx of Q69.__ (depending on digit) and have never had an issue getting reimbursed.
When you look up code 26587, reconstruction of polydactylous digit, soft tissue & bone, directly underneath in parentheses it states For excision of polydactylous digit, soft tissue only, use 11200.
