Please help me to code the below document.

Chief Complaint: Intrapelvic Abscess

The indwelling IR transgluteal drainage catheter and right buttock region were prepped and draped.
5ml 1% lidocaine for anesthesia.
Under fluoroscopic guidance the indwelling catheter was injected with gastrograffin contrast.
Fluoroscopic spot films were obtained in the AP, lateral and oblique projections.
The previously identified perirectal abscess collections has considerably decreased.
There is now definite fistulogram action to bowel.

The indwelling perirectal abscess drainage catheter is removed.
Over wire under fluoroscopic guidance.

Impression:
Contrast injections of perirectal drain revealed no significant cavity or fistulous connection to the bowel.
The catheter is then removed over wire under fluoroscopic guidance.
