Remove g-tube, closure of track

I am looking for codes for removal of G-tube. Is this to be considered and E/M visit and also closure of the fistula track. Any thoughts would be greatly appreciated.
 
we use 43760 if we replace the g-tube. If you're removing it and closing it surgically, I'd use 43870 with the dx code 686.9. Hope it helps!
