Repeat screening colon 1 yr after piece-meal removal of polyp

Patient had a screening colonoscopy for personal history of polyp 1 year ago. Adenoma polyp removed piece-meal with hot snare and completely removed. Was billed 45385-33 and Z12.11 used. Came back this year, indications were personal history of polyps, Colon performed for screening. 2 polyps found, one was adenoma removed with snare and the other hyperplastic removed with cold biopsy. What is the correct way to code this? Patient not due for another screening. Would you use the polyps first and personal history last. Or since Dr said Screening indication should you code as a screening and use Z12.11 and 33 mod again?
