patient had painful hardware in the hip, so doc replaced the gamma nail. would i use 20680 for this? any help is appreciated!!

I would use 27236 (Open treatment of treatment of femoral fracture, proximal end, neck, internal fixation or prosthetic replacement).

I would not code the removal of hardware. Since he is replacing it, removing it is part of the package.
