Replacement code for H2019, payer Medicare?

We have care coordinators that have been sending claims over with H codes to bill Medicare, this could is not accepted by Medicare. We are wondering if there are any payable codes H2019 could be replaced with?

If you read the heading prior to the H code listing, you'll see that those codes are for Medicaid, although some commercial payers may accept them. Medicare won't pay on them; typically there's a G code or a category 1 CPT code that will be acceptable to Medicare.
