I have been noticing that when residents see patients their signature isn't dated or timestamped, does that count?
 
This is what CMS says: Medicare claims reviewers look for signed and dated medical documentation of those responsible in providing care for the Medicare patient to meet Medicare’s billing and coverage policies’ signature requirements. If the entries don’t meet the signature requirements, we may deny the associated claims.
This does not state only the supervising physician or billing physician, it's anyone providing care. You may want to check with your Compliance team.
