Pt. came in for tx of plantar warts, this was to be the second tx but when the tech saw the pt the warts were resolved. How would this be billed? Would you use the dx or Plantar wart or should we use something else.

I usually just bill 99212 with a follow up code or with just the wart diagnosis or better if use both codes.
 
If the warts are no longer present and no longer require treatment the you use the Z09 for follow up. The visit level will be dependent on th documentation.
