Wiki Resources for Billing Out-of-Network?

Does anyone know of any good resources - webinars, conferences, etc. - that assist with billing out-of-network? I work for a DME company and we ship products out-of-state, and we often bill out-of-network because carriers in other states won't contract with us. We run into all kinds of challenges such as insurance sending payments to patients, insurance refusing to communicate with us because we're out-of-net, insurance refusing to accept an appeal from us, etc.

Any references on how to learn how to work out-of-net?
