This is an exerpt from a surgical note:

"In preparation for hi surgery, an arterial line was
attempted in the right radial artery; however, the needle went in but the
wire could not be advanced. When the anesthesia team tried to pull out the
wire, part of it was torn and was left in the wrist. The rest of the wire
came out."

This is from the vascular surgeon that was called while the patient was in the OR for the original surgery and the wire was successfullty removed. We are looking for an appropriate diagnosis to attach to the procedure to remove the retained portion of the wire. We have come up with several different combinations but we are not pleased with any of them.

What combinations did you come up with

What combinations did you come up with, and why aren't you satisfied with them?
