Doctor said performed revision TKR with patellar resurfacing and revision tibial polyethylene. He performed medial parapatellar arthrotomy with reaming. The Wright Medical implant was cemented. The knww was lax in flexion and extension so the 14mm spacer was removed and 17mm medial pivot implant was snapped into place.

Please help with code!
