My question is, If an outpatient hospital claim is billed with 4 units- Revenue Code 360 - CPT Code 49650. Is there documentation somewhere that says the 4 units are reflective of the charges under Revenue Code 360, NOT reflective to the CPT indicating 4 units of procedure code 49650?
