On the examples below regarding the ROS would the statement suffice for a complete ROS:
REVIEW OF SYSTEMS:
Review of systems x 12, denied except for acute gouty arthritis.

or just

Review of systems x 12
 
Simply stating ROS x12 would not be sufficient in a complete ROS. Remember, the ROS, should be pertinent to the patient's presenting problems, and the ROS are questions asked to the patient.
A full ROS for a patient who has a headache, as an example, may not warrant a full ROS.
 
Another thing to keep in mind is that there are 14 systems, not 12. If a provider uses something like your example, there's no way to know which 12 he reviewed. I wouldn't give credit for full ROS for this.
