Can you bill for a Revision of Transmetatarsal Amputation? If so what would you bill?

I would bill 28810 with modifier -76 if I was done by the same doc within the global.

Be ready for the insurance company to be stupid. I hate to be so blunt, but do they think that the doc removes the entire metatarsal bone up to the tarsals? I would not be surprised if they think the procedure can't be done again because it's already been amputated. Keep the op close to you and get ready to send it in with an explanation on why it was necessary to repeat the procedure.

My guess is more bone became avascular and necrotic necessitating the repeat amputation.
