Wiki Revision TMA with Primary Closure

First procedure was Transmetatarsal Amputation CPT 28805 (not closed on this date due to extensive infection)
with Deep I&D Foot CPT 20005, on same date.

5 days later: Revision of TMA with Primary Closure.
Physician used saw to remove small amount of bone from each remaining metatarsals, and closed the wound.

What do I bill for the Revision, and Closure, if anything?
