Patient had a total hip arthroplasty years ago. She fell and Dr did a "revision".

He replaced the femoral head. He did not replace the femoral stem. He then removed the cup and replaced with a polyethylene cup.
I am confused on this. I don't think This wouldn't be considered a full revision . Or am I wrong?
Can someone help me with the correct code to use?
