Can anyone help me with coding this:

Dictation reads:

Radiofrequency ablation of the peripheral nerve around the scar from the jejunostomy tube

Mr xxx come into the pain clinic today for a radiofrequency to be done for his intractable neuroma and neuroma pain. This is to be done under conscious sedation. The jejunostomy scar was anesthetized with 1% Lidocaine and a radiofrequency needle with 10 mm open tip was then introduced surrounding the scar. At various lengths a total of 4 needles were used each time and a total of 8 lesions were planned. Each needed was then placed at different depths and the radiofrequency applied at 80 degree cGy for 80 seconds. The needles were then rotated 180 degrees and radiofrequency reapplied. A total of 8 lesions were created and he tolerated the procedure well.

I think this should be billed as CPT 64640 but not sure if this is correct and also not sure if it should be billed multiple times and if so how many. Doc says it needs to be billed 8 times.
