Hello all

I need help with Rural Health Clinic billing. We are a hospital based RHC and I need to know about T1015 HCPCS and when this is used and on what claim format does this go on (UB or 1500)
I am so confused as we get an all inclusive rate with Medicare and Medicaid for our visits.
I have reached out to TMHP and to our Medicaid rep but I cannot get straight answers.

Any advice?
