Received op report to code and procedure is listed as Rhizotomy. Here is the segment in question:

I made a stab wound incision slightly paravertebral and in between L5-S1 and L4-5. This allowed me to apply the dilator system and a scope was applied over the transverse process juncture with the facet joint at L4-5 on the left side, documented by fluoroscopy. Under direct visualization I identified the median branches and coagulated and cut them.

I am new. I'm being told the correct cpt is 64772? Would someone please help me.
