Can anyone give me some clarification on the collection of HCC Codes for CMS from SNF patients from the physician E&M visits? I know SNF is not an approved provider but what about the Physicians or NPP that see the Healthplan members monthly. Can those records be used for HCC code capture for RAPS?

Of course I will need back up on this in writing from some source.... This is maddening!
