We need some coding help for a delivery.

Our patient came to L&D via wheelchair. When she stood up to transfer over to the delivery bed her baby fell out onto the floor. The RN picked the baby up and put him in the warming bed and NICU took over, etc. Our physician came into the room just moments after the birth, delivered the placenta and repaired a 2nd degree vaginal laceration.

Total antepartum care was provided by our physician and she wants to bill global 59400. Some of the coders here feel that no one really delivered the baby and we should bill the AP care (59426), vaginal repair (59300) and PP care (59430) only.

We know that sometimes if the RN truly catches/delivers the baby then the physician can bill the delivery/global but in this case the patient really ?delivered? herself.

Can you please give us some insight on how this should be billed? Thank you!!!!
 
your coders are correct. the mother delivered with no outside help. case happened in our hospital recently where mother went to use restroom and spontaneously delivered baby. Unfortunately, physician cannot bill global delivery services but I think you can bill for delivery of placenta along with repair and ante/pp care.
