Wiki RN Doing Chemo IV's: Supervision?

We have an RN doing Chemo and other complex biologic infusions once a week in our physicians' Infusion Clinic. Are we to treat her services like "incident to", insofar as supervision? (We realize we cannot bill for the RN's services to any carrier.)

Can the infusions be billed under the patient's doctor who ordered the infusion, as long as the doctor is in the suite when the infusion occurs? If that patient's own doctor is out that day, must we bill under a supervising doctor who is present? Is it OK to allow the RN to continue the infusions even if the supervising physician steps out to attend hospitalized patients? Thanks for any/all help.
