If an rn does a treatment in the office or gives an lhrh injection and no e&m code billed. Is a cc or dx needed in the documentation
 
When a RN performs injections following the providers orders, she would use the diagnosis code provided by the provider for treatment of the patients condition. Each medical record must support the services rendered with supporting diagnosis.
A RN cannot diagnosis a condition, but she can diagnosis sign & symptoms
