If dr ordered a rocephin injection but pt didn't get it until 2 days later and dr was not there, can it be billed under dr name because he ordered the injection?
 
No there must be a provider in the office at the time of the service. The ordering provider NPI goes in filed 17 with the qualifier for ordering provider, the provider that is on site will go in filed 24J. If this is a physician office, there must always be a provider or qualified NPP when services are rendered. If you have MAs and or nurses only on site then there can be no services billed.
